When nursing clinicals come around, it can be awkward being a new student, shadowing your buddy nurse while they give/receive shift change report. What do you write down? How do you even give report, and what do you include? Usually you won't be called on to give report until your practicum rotation, but you will get report on your assigned patient(s) in any nursing clinical you have, so I made a simple report sheet that i used as a student, and will likely use as a nurse in my career. It is important to have some idea of what you need to know, therefore, this sheet helps you to organize some simple information on your patients so you are not completely lost if your nurse, someone else on the care team, or authorized family asks you for simple information. You can omit any information that is not relevant, add more information that is not on the sheets, or personalize them to each patient, but if you have no idea what you are doing, they are a good place to start!
Generally, the two patient sheet is going to be a better option for a new nursing student, since you will likely only have 1 patient you are assigned to, and then you have an extra "patient" in case you mess up on your first, or if you need more room! This is also a useful layout if you are working in an ICU setting, as you should only be assigned to 2 patients anyway. The information included on the sheet is all information you should be sharing with the oncoming nurse, and it is a good way to organize the basic patient information, as well as helps you plan out your shift when you have all of your patients in the same place.
If you are working on a general unit or if you just prefer, this layout includes 4 patients. This one was especially helpful in my practicum, as I was responsible for more than one patient at a time, and it was useful to have each of my patients on the same piece of paper in front of me.
Giving and receiving report is not something you need to be scared of or nervous about. And the most important thing to remember is to keep it basic and organized. Formatting a report sheet for yourself, or using mine, can help you to get your information together and to make giving report at the end of your shift much more doable, and receiving report much less overwhelming. I hope these report sheets help any new nursing students, or nursing students that don't know where to start to strive for organization in their clinical setting experiences.
